Question: I have an external Hemorroid. I am on many drugs including Coumadin. I have no bleeding but plenty of anal rectal pain.
Should I call my Coumadin clinic about lowering or ending my coumadin for a few days. Treating with warm baths, witch XXXXXXX and Prep H last three days no improvement.

Well, at this outset, since there's no active bleeding, I'd recommend checking the PT and INR levels at first...If it's between 2.0-3.0 then not to worry.

Yes, please continue taking Sitz bath and Prep-H as you're already taking.

Additionally, follow these bits of advice:

Hard motion and constipation can cause excessive straining which in turn results in the injury of the anorectal region. Since the hemorrhoids are already diagnosed, perhaps take Syp. LACTULOSE 15ml once daily at bedtime and observe how you stay (dose can be further escalated up to 30ml thrice daily based on the initial response)...

Apart from this, please do not strain while passing motion...

Drink plenty of water, have plenty of fresh green vegetables, simple homemade foods, salads, curd based diet, avoid any form of spicy food...

In case the pain doesn't reduce despite this, it'll be better to get it examined clinically by a Gastroenterologist or a Surgeon for further assessment...
