In the picture...the hemmeroid is the small bump and then there’s a bigger one next to it? Is it common or what are options of what it could be? Could it be serious? Need to be drained or is that what a thrombosed hemmeroid is?

Detailed Answer:
Thanks for your feedback and photos.
I can see a swelling, edema, redness and a mass like lump away from anus.
The probable diagnosis is:
Ischio-rectal fossa abscess.
Fistula-in-ano appears like this.
Local boil.
This is not common.
Can be serious hence needs correct diagnosis by proper examination; MRI helps to know the extent and diagnosis in doubtful diagnosis. Please discuss with your Doctor, preferably a General Surgeon/Proctologist.
Thrombosed hemorrhoid will be at the anal verge and not away from it.
I hope this answers your query.
You may add new photos taken from a distance to show the anus and the lump to have more clear picture.
