I Have Been Suffering From Sleep Disorders Issues Since Last

Question: I have been suffering from sleep Disorders issues since Last 4 years.
I have consulted many psychiatric doctors and complete their given courses. But no effects. After that one of my friend suggested me to consult pulmonologist doctor, than i have visited one of well known pulmonologist , he suggested to do sleep study and test required serum cortisol and TSH , I have tested please find report and Please helps on this.
