                                            Question : I have had bilateral kidney pain on and off for about 5 years. With the kidney pain, there is no fever and no blood tested positive in the urine. About 5 years ago, I had some type of diagnostic imaging which revealed kidney inflammation, but eventually resolved. No cause was identified. Interestingly, I have also suffered pleurisy of the lungs wherein I have required hospitalization and needed about 1 quart of fluid drained off of each lung. Again, so cause was identified. More recently, I have noticed that my joints are stiff and I have chronic aching in my upper and lower spine which has led me to believe that this might be arthritis. It runs in my family, both rheumatoid and alykolosing spondylitis. I have had general testing for rheumatoid that were negative, but never specific through a rheumatoid doctor. I am told the testing is much different. So my question is what testing should I have done specifically both blood work and diagnostic imaging for the kidneys.

you should get an HLA B27 test for ankylosing spondylitis, and maybe an MRI for the sacro iliac joints.
for the kidneys an ultrasound for the kidneys should be sufficient.

                                    I uploaded labs. Is there anything g concerning. I have quite an elevated CA125. Would arthritis affect that?
Also, I am surprised that I wasn’t checked for the hlab27. Is that something that if you can be negative for and later in life be positive?

there isnt much concerning as far as arthritis is concerned.
I couldnt find any test for hla b27, this result generally does not change, however if the method of carrying out the test changes, the result may vary.
plus it is not specific for ankylosing spondylitis, however if positive correlates better with the disease.
