Question: I have on two occasions experienced tinnitus that resolved with antibiotics. About a month ago I noticed cackling in my ear with unexpected noise. Water running is a big one. The entire time I am in the shower my ear snaps and crackles. Walk out the door and the crickets cause my ear to react. Otherwise no tinnitus. Any thoughts.

As per your description, it seems like a case of Otitis media with effusion (infection of the middle ear causing fluid and mucus built up inside). This also causes partial blockage of the Eustachian tube (it's a small tube which drains fluid and secretion from our ear to the throat), which is why those snapping/cracking/popping like sensation arises.

To confirm the diagnosis, direct clinical examination by an ENT-specialist is required. Based on the findings further prolonged antibiotic treatment along with decongestant therapy should be advised. Since it's happening on repeated occasions, there may be a requirement to escalate the antibiotics also.

Additionally, please refrain from having any cold beverages, extreme sour foods as they can further worsen the symptoms. Avoid going to any dusty/overcrowded places for the time being.
