I am so sorry to hear about this. In most cases, this is benign and requires no detailed evaluation or investigation. The most common cause is the pooling of blood which often occurs in the upright position and relieved by lying flat.
In other circumstances, this could be an infection. If you are sexually active, please let me know. Also, if you have a urethral discharge or redness of the testicles, kindly let me know.
In the interim, I encourage supportive underwear and avoidance of prolonged upright positions.

MY recommendation would be to have supportive underwear and avoid repetitive manipulation of the scrotum.
