The bulge you mentioned could be due to an anal fissure or impending piles...this can be due to several reasons starting from hard motion and long term constipation, or some other pathology...
The uncontrolled blood sugar can prolong the healing process, so proper medical evaluation is recommended...it'd be better to get the swelling examined by a General Surgeon or a Gastroenterologist for the necessary diagnosis and further specific management...
Meanwhile, you should start taking 'Sitz bath' thrice daily (sit in a tub of lukewarm water putting some salt in it)...You can watch a tutorial video of Sitz bath online for proper techniques...
Along with it, most importantly, please don't strain while passing motion...
There may be some relief by applying ointment containing Lidocaine and Hydrocortisone (eg. SHIELD ointment for local application outside the anal canal only), but it's only for a short term relief...You need to get it checked up at first by either a Surgeon/Gastroenterologist for specific management...
For long term relief:
