Brief Answer:
There can be several causes, require further evaluation.

Detailed Answer:
Hello,

I have gone through your query and here is my advice.

As per your history, there can be several causes for that, but most commonly, it may be due to muscle spasm or strained muscle.

For further assessment, I would suggest you to consult with spine surgeon for detailed clinical examination. He may procure MRI back with whole spine screening, nerve conduction studies as per evaluation to rule out nerve compression due to herniated disc or arthritis. Further treatment mainly depends upon the underlying condition.

Mean while, continue pain medication and muscle relaxant. You can apply diclofenac gel or ointment locally. Gentle application of heating pads will also help. Avoid forceful bending and take proper rest.

Hope I have answered your query.
