Question : I have this blotch on my arm for about 6 months. it itches but doesnt hurt. can you tell me what you think please?

Hello and welcome to "Ask a Doctor" service,

Itchy skin lesions can be due to dermatitis or fungal infection. Lichen sclerosis may happen if an itchy lesion is chronically scratched (it causes the skin to be rough).
I would recommend you to use a combination of a steroid and antifungal. Betamethasone thin layer can be applied on the lesion and miconazole or ketoconazole cream over it twice a day for around 2 weeks. Betamethasone would help with dermatitis and lichen sclerosis and the antifungal will help with the fungal infection.

Normally, the lesion should start to fade after 5 days of starting the treatment. If the lesion is not getting better in around a week of having these two creams applied you will need to have it checked with your doctor. Sometimes, lesions that are not regressing in months and do not respond to local treatment need to be biopsied for definitive diagnosis and treatment.
