Detailed Answer:
Hello dear welcome to Ask a doctor's service.
I read your query and here is my advice.

I see the test results you uploaded and you are having very low levels of vitamin B12 which can be the reason for extreme fatigue , low back pain and sleeping issues.
I think taking vitamin b 12 injections is necessary and you need to start it as soon as possible. This therapy will help the symptoms and may bring the red cells count back to normal.
The ers is also high that may show high inflammation or rheumatological issues.
You may need to do other tests such thyroid balance , T3,T4,TSHand aTPO .
Also you may need to do rheumatoid factor test
Also you are having high cholesterol levels and a low fat diet may be recommended initially.
Hope I have answered the question .

Hi Doctor,
My TSH=4.09, T3=108.3, T4=7.08
Can you please tell me the name of B12 injections and the frequency to take these?
Recently I am having chills at night with no fever though the temperature here is >40 degree celsius. Also, the roof of my mouth is swollen and I have ulcers in mouth. Please advice.

Detailed Answer:
Hello dear and thank you for asking again .
The thyroid hormones levels you uploaded are within normal range.
The ulcers in mouth may be a symptom of low vitamin B12.
You can start taking cyanocobalamin injections every three days for two months then one every week for another two months.
