Question: I recently took a health checkup and there are few deficiencies in vitamin d and b12. I want to know how to balance them.

I read your query and here is my advice.

I see the reports you uploaded and almost all parameters are normal except vitamin d and vitamin B12.
These deficiencies are caused because of your vegetarian diet. These vitamin are usually found in animal origin food.
You can take supplements in order to increase these levels.
You may need to take vitamin D 2000ui once daily and vitamin B12 250mcg daily.
You can check the vitamin levels after a month and see if they are increased

Vitamin b 12on an empty stomach and vitD with meal

You can have vitamin B12 with an empty stomach or at least 30 minutes before meal , in the morning or before dinner.
Vitamin d can be taken with the meal in lunch time.
