Question : I submitted a case study tge other day but i never heard back from anyone. i need help with the case study attached tothis inquiry.

I have gone through the case study attached.

1. The diagnosis would be a coagulation cascade disorder, possibly hemophilia. The PT is normal but APTT is raised, hence the diagnosis. The clinical features correlate.

2. Intravenous replacement of the specific clotting factor is the preferred mode of treatment in severe cases. Anti-fibrinolytics and desmopressin might be required.

3. Unexplained and excessive bleeding from cuts or injuries, or after surgery may be observed. Many large or deep bruises may be observed. Unusual bleeding may occur after vaccinations. Pain, swelling or tightness in the joints may be seen. Blood may sometimes be observed in urine or stool. Nosebleeds may occur without a known cause

4. Intrinsic pathway is involved.

So...would you say that the pathways involved in this diagnosis are (1) Extrinsic pathway -which activated the clotting, and (2) Intrinsic pathway-which formed the clots?

so what else can we use to help treat the baby? Can we use Vitamin K ?

FFP transfusion may be given

Detailed Answer:
The primary thing would be to identify the specific factor that is deficient (through proper test) and to substitute it. Other steps are already indicated in my previous response. Sometimes fresh frozen plasma is tranafused since it contains coagulation factors.
