Question: I suffer from anxiety and panic attacks. I want to get back into working out. What will happen to my heart, breathing while working out and also having panic attack. I have high anxiety about working out because of what could happen...heart attack, hyperventalating etc.

You are a 28 years old male suffering from anxiety and panic attacks. You want to get back to working out but you are having anticipatory anxiety that what will happen to your heart or breathing. You are also having anticipatory anxiety about having panic attacks.

See anticipatory anxiety is a common complaint in patients suffering from anxiety disorders. This is good that you are having motivation for work out but anxiety is stopping you.

I would advise you to go for Psychopharmacological treatment as well as to try Psychological methods. But psychological methods are useful only when you have drive for that. For that you have to go for proper pharmacological treatment as per my opinion.

- First of all get your complete blood counts done. Anaemia or low haemoglobin may be a reason behind the symptoms of anxiety. Panic is also very common in anaemia.

- Apart from blood tests get your Thyroid levels done. Hyperthyroidism is a common reason of anxiety and panic like symptoms.

- Get ECG done to rule out any cardiac cause behind your symptoms.

Most likely all tests will come in normal range. Next step in treatment is medicinal treatment. Consult a good psychiatrist for expert management.

-Medicines like SSRIs as Sertaline, Fluoxetine, Escitalopram are very useful in anxiety disorders. They can effectively control anxiety and also prevent panic attacks. Low serotonin levels in brain are the reason behind anxiety symptoms and these medicines help to improve the levels.

- For episodic control of anxiety and for control of anxiety and panic symptoms medicines like Benzodiazepines as Clonazepam, Alprazolam etc are used. These medicines help in early relaxation. But continuous long term use of these medicines is not advisable because of dependence potential.

- These medicines will help to improve your symptoms of anxiety and will increase the confidence.

Apart from medicines non medicinal treatment like Relaxation exercises as progressive muscle relaxation techniques, breathing exercises and yoga help to improve the symptoms of anxiety. Behavior therapy with help of a trained psychologist will help to improve the faulty cognitions and this will increase your confidence.
