I Take Methadone For Chronic Pain ( 10 X 10mg

Question : I take methadone for chronic pain ( 10 x 10mg daily, I've been on this dose for 10yrs). I started Levaquin today for sinus infection, I had to take EKG to screen for Long QT Syndrome because the 2 meds cause major possiblities for Long QT.
Would it be safe to add voriconazole as well? It has moderate interaction with Levaquin for Long QT, does that increase the possibilities or is it not an addition factor?

Regarding your concern, I agree with you on the fact that levaqui and methadone both increased QTc interval.

But, voriconazole may interact with both these drugs by increasing further the risk of QT prolongation and thus cardiac arrhythmia.

Besides, it may increase the plasma levels of methadone, thus increasing its adverse effects.

So, I would recommend avoiding voriconazole.
