I Was Checking My Vagina Doing A Self Exam And

Question: I was checking my vagina doing a self exam and deep into my vagina on my lower left side I found a lump that’s almost the size of a skittle or mnm candy. It’s is very firm, not movable , and I can feel it is fully round. It’s also painless.I had a check up last week w/ a gyno where they checked me because I had a little bit of pain. But she didn’t find anything and told me everything looked normal. To my XXXXXXX last I night I happen to check and found the lump very easily. I don’t know how the doctor missed it.

Since it’s very deep inside my vagina there’s no way I can see it. What other possibility besides malignancy could be possible? I will try to get an appt on Monday but meanwhile I am trying to see what possible outcomes are possible. I cannot think of another possibility besides malignancy b/c of its location. I have no discharge, bleeding. Some pain during sex which I’ve had on and off for years and I have bladder cystitis.

Brief Answer:
? vaginal mucous cyst

Detailed Answer:
Hello,
Thanks for the query,

From the above description of the lesion, it appears that it is a vaginal cyst that is often soft to firm in consistency and can arise anywhere along the mucosa of the entire length of the vagina. This is usually asymptomatic and benign and is removed surgically if symptomatic.

However, this is just a differential and one needs to have a rectal ultrasound or an MRI pelvis or a focussed per speculum examination to establish the diagnosis.

Thank you for your response. And yes i know only testing will let me know what this is.
Ive been to my gyn for my regular checkups and i just had my daughter 10 months ago, so id like to think they would have seen this if it was there. Hopefully it's just this last doctor who missed this.

I know everyone is different but what are some symptoms that would usually show In early stage and late stage vaginal cancer along with a lump?

How long does it usually take for a lump to develop?
Thank you.

And would anything show up during routine bloodwork if this was malignancy?

I have one additional question so for there to be no confusion i will rewrite them here:

-What are some symptoms that show in an early stage of vaginal cancer besides a painless lump?

-How much time does it usually take for a lump to develop?

-If it is malignancy, could you have a lump and still be at a stage 0 or stage 1, or does a lump automatically mean that malignancy is at a further stage?

-Would bloodwork show any abnormal levels if there is malignancy going on?

Thank you doctor.

Detailed Answer:
Hello,
Vaginal cancers are uncommon in your age group , however if its a malignant cancer then there would be satellite mucous growths as well and the rate of increase in size would be fast and would not be slow progressing. Also there would be associated weight loss, cachexia and generalised debility.
The routine blood work doesn't show malignancy however an infection can be pointed out by total leucocyte count and also the lymphocytopenia. There are tumour markers which need to be done for suspecting cancer.

Just went today to have a coloscopy and gyno could not get a sample biopsy because of location of bump. She mentioned she does not think it’s cancerous b/c lump is movable. But she was not able to see it or get a biopsy. She said to reschedule in a month as I must go under anesthesia to get a biopsy and even then she is unsure if she can get it. If she is unable to get it, what other procedure could be done and what kind of specialist would I have to see?

She was adamant she doesn’t think it’s cancer but I know only a biopsy will tell me the answer.

the doctor was unable to even use the cytoscope b/c of the lumps location and was trying to do procedure just by feel with the help of another doctor. Without seeing what she’s doing it will be difficult so that’s why I want to know what other procedure can be done and by who. Thank you.

What I mean by who I mean as in what kind of doctor, a surgeon or a gyno-oncologist would be best to get this biopsy since it’s in a tough spot. Had ultrasound done and cervix and everything else looks good.

I just looked into urologists and it seems like they could use the same camera they use for cytoscopy to get a look inside my vagina and get a good look at the lump. Do you agree? I will call in the morning Sorry for all these questions. I’m sure you can understand my urgency. Thank you.

Detailed Answer:
Hello,
I am really surprised here. Ideally an ultrasound transvaginaly or even a rectal ultrasound can easily delineate the plain between a normal tissue and the vaginal mass / cyst.
Yes, vaginoscopy is an option which can help reach the lesion and then vaginoscope guided biopsy can be attempted.
But before that why is that your doctor not advising you an MRI pelvis and vagina , which can help differentiate between a cystic or a solid mass .

I did get a vaginal sonogram prior to getting the coloscopy but they were not able to see the lump once again because of its location. I am surprised she did not tell me to get and mri. Today I called and was able to get an appointment w/ a urologist for next week so I am hoping he can further guide me throughout this process. Thank you for your time and help doctor.
