Question: I would like a doctor to explain the comment in my semen analysis “ 1-2 spermatozoa per 7-8 high power fields with abnormal morphology and a few with sluggish motility “ . Should I be worried or is this normal?

Hi,

I saw your semen analysis report.

Few sperms in a field that too abnormal ones means chances of conceiving normally are very low and almost impossible.

There are no effective medicines to improve these parameters.

You need to undergo procedure like ICSI ( Intracytoplasmic Sperm Injection) to conceive and have a healthy baby.

I am not familiar with ICSI but am familiar with IVF. What are the difference between them and do I need to do both? Also I wanted to know what are my chances of conceiving with ICSI with low sperms?

IVF is a procedure where in sperms are left around the egg and expected to fertilize egg by themselves. when Sperm counts are normal, this IVF can be done.

Egg is held and injected with good sperms in cases of ICSI. This procedure is for men with low sperm count.

ICSI will be helpful for you to get pregnant.

So in my situation does it mean I do have sperms or does my analysis show no sperms?
I met my Dr. today and he said he wants to check my (FSH,LH,prolactin and testosterone levels) if levels are low this could mean my pituitary gland isn’t working very well and I need to take medication to balance it out so that I can produce more sperms. I was wondering if this could mean I will be able to have children on my own without ICSI.

I would like to note that I don’t have any medical problems and don’t take any medications. My Dr. also examined my testicles today and said my left one is a bit smaller than normal but my right one is normal in size.

Hi,

This report is severe oligospermia. Not azoospermia.

But the counts are so low that it might soon slip into azooapermia. Please go ahead with ICSI as soon as possible.

High FSH and low testosterone is because there is no production of sperms from majority of portion of testes.

Unfortunately I don’t live in XXXXXXX I am trying to look up for a reason why my testosterone is very low and because my FSH is high, could this mean I have primary hypogonadism? And how can I rule out if it’s congenital or acquired? I looked into klinefelter syndrome but it was said that the testis are very small about 2 cm length and 1 cm width which is not my case however my left one is considered small but still bigger than 2cm. Do you still think I should rule out klinefelter syndrome with blood test?

This is likely to be primary hypogonadism.

You can undergo genetic tests like karyotyping and CYP gene microdeletion to understand if its congenital or acquired.

Better to rule out Klinefelter syndrome also.
