Question : I've received my health check report yesterday. I would like to consult the doctor to understand the report and seek advise.

Detailed Answer:
I have gone thru your health check up reports. Though some of the reports are marginally out of range, but I do not think these as clinically significant, particularly seeing your other reports. However, I think, that the following reports should be particularly taken care of by you.
Your vitamin D is low, though shown in refrence range, still you may take vitamin D at least for some time.
Your vitamin B12 is definitely low. Hence you are advised to take vitamin B12 to make for its deficiency.
I think, these are the remedial measures, you should take, as far as, your health check up reports are concerned
Symptoms of Vitamin B12 & Vitamin D deficiency may be variable & mostly depend on duration & severity of deficiency.
Symptoms of B12 deficiency may be general like fatigue, weakness, dizziness as well as shortness of breath etc. Neurological symptoms like numbness & tingling in hands & feet, difficulty of balance & co-ordination may be also there. Loss of appetite, ulcers of tongue & oral cavity are other presentations.
Similarly in vitamin D deficiency bones & muscle pains, muscle cramps & joint pain may be there. In severe deficiency weakening of bones( osteoporosis) or soft & deformed bones in children(rickets) may be there.
The required dose of B12 in your case may be 1mg of B12 daily for 1-2 months & then 250 microgram daily for some time as a maintainance dose depending on your symptoms & blood levels of Vitamin B12.
Similarly Dosage of vitamin D in deficiency may be 6000-10000 IU daily for 3-6 months or 60,000 iu weekly for 3-6 months, followed by a maintainance dose of 1000-2000 IU daily or 60,000 IU fortnightly or monthly depending on the levels of vitamin D in your blood.

I used to take Calcimax Forte, 1 tablet every night till Nov'24. I've resumed the same after this Healthcheck. In addition I've started taking Seacod, 1 capsule in the night. Other than these two, no other medicine or supplements.

Both calcimax forte as well as seacod softgel capsules are health supplements. However, these do not contain adequate doses to treat your deficiency of vitamin D as well as of B12.
There is no need of a prescription for these. Both these are easily available OTC without a prescription. These are available in varying doses under different brand names. Even generic formulation under vitamin D & Vitamin B12 are available with the pharmacies OTC & NEED NO PRESCRIPTION.
