Question: Hi Dr. XXXXXXX I am the patient you have been helping over the last few days abot me having nausea for the last 12 days. Our previous conversation was finished so I used the link you sent me to ask another question.
Yesterday I was feeling about the same. I had taken 300mg of Zantac again yesterday. I still had some nausea but not quite as bad. I did not have a very good appetite but I was able to eat. I felt like my body temperature was up so I checked with the thermometer but I had no fever.
Today I have not really had any nausea so far. I have taken my morning dose of Zantac. I don't have the best appetite again but I am able to eat. My body temperature feels higher to me again which is causing me concern. I just checked it and it is 99.2 (I use a thermometer that goes in your ear). The ER doctor had told me to come back to the ER if I get a fever. I am not sure what to do. My temperature is not really high but higher than normal. The funny thing is that I don't actually feel as bad today except not much appetite and higher temperature. I would really appreciate if you could let me know if you think I should return to the ER. What temperature is high enough to consider it a fever and have to go back?

Its good to hear that you are recovering every day.
As per the temperature is concerned yes you do have fever however as it is less than 100 degrees Fahrenheit you can take a tablet of Tylenol.
Appetite would return back slowly. So don't worry too much about that. Take lots of fruits and vegetable to provide you with natural energy boosters. Drink fresh coconut water.

I was wondering if it would be possible for me to ask you a question about my 4 year old daughter?

Over the afternoon we were at the beach. She seemed to be completely fine while we were there. When we returned and started to get ready to go to a restaurant she said she felt tired. We went out to eat and she had very little appetite and was very tired. She felt warm to me so we brought her home. When we got home I took her temperature and it was 102.4F. She said her tummy and her head hurt. She also mentioned her hand feeling weak. I gave her 7.5mls of Children's Tylenol 5 hours ago and we put her to bed (she is sleeping with me). I check her body every so often and her body keeps feeling hot to me. I just checked her temperature and it is 103.3F so I gave her another 7.5mls of Children's Tylenol (it has been 5 hours since her last dose). I asked her if anything hurt and she said just her tummy. She is an otherwise healthy child.

Since Monday I have had a very low grade fever on and off (it has not reached 100F that I know of). The highest I checked was 99.6F. My husband has also felt warm and achy on and off since Tuesday but never actually registered a fever on the thermometer. We are not sure if we have been fighting something.

Anyway my concern is that the first dose of Tylenol I gave my daughter did not bring the fever down. I don't know if I should be taking her to the hospital or keep treating her at home. I have heard that you can treat at home if the fever stays below 104F and goes down with Tylenol, but I am not sure if it went down at all.
Any advice is appreciated. Thank you

Hello again my dear patient,
If temperature of patient is above 100 degree f you have to get i.v shots of paracetamol as just tsking it orally won't help.
As her tummy is hurting very likely she is having intestinal infection causing inflammation.This inflammation causes fever.
Suggestion,
Take her to yhe nearest hospital and get a stool test done.
Proper antibiotic therapy should be taken with parentral paracetamol.

Thank you so much again for your help and advice.
I have been asking you questions for the last week about me having a possible ulcer. I will be seeing my doctor tomorrow. I think I understand what is involved to treat an ulcer. If it ends up that I only have gastritis and not an ulcer, would it be best for me to be taking Zantac or a PPI like Prevacid? Also, approximately how long does it take to recover from gastritis if you are taking the medication, maintaining a proper diet and trying to remove stress from your life? I have heard it can be about a 6 week recovery. Would you say that is correct?

Initially, you should take a PPI after few months when relieved can switch over to H2 blocker.
4 to 12 weeks depending on the response of the body of the patient.
Yes, it can be there in 6 weeks also if everything is followed properly.

I saw my doctor today. I told him that for the first 5 or 6 days of taking 300mg of Zantac my nausea had improved. However, over the last 2 or 3 days of taking it the nausea was becoming more frequent again. He told me to stop taking the Zantac and has put me on Dexilant 60mg for 10 days. He said if I have gastritis or an ulcer this medicine should help me feel better quickly. When I see him in 10 days he will see how I am doing and decide what to do next. I took my first dose of the Dexilant 9 hours ago. I am not sure how quickly it should be working. I noticed that my nausea was not too bad for about 5 hours. It started up for about an hour but was not too bad. It has not been too bad for the last couple of hours. So like I say, the nausea is not completely gone today but it did feel better than the last couple of days. How long should it take for the Dexilant to work and for me to notice a difference with the nausea? If it is working, will the nausea completely go away or is it possible I will still have it? I am the type of person to worry so if I still have some nausea after a few days I will probably assume the medicine is not working.

If my doctor feels the medication is working, how long should he keep me on it? I think in your last message you said about 4-12 weeks?

Hello, again dear patient,
Happy to hear that you are feeling better,
I told him that for the first 5 or 6 days of taking 300mg of Zantac my nausea had improved-This is an H2 blocker and not meant to treat nausea.
Dexilant is a proton pump inhibitor which are better drugs to control acid peptic disorders as compared to H2 blockers.
It will take a minimum of 3 days, however, an over the counter anti-nausea medication if taken along with it like Zofran 4mg would help you to recover much faster.
If gastritis is chronic or you have ulcers then the drug will only suppress symptoms for some time and an endoscopy would be done.
If the medication is working then he would keep you on it for a minimum of 21 days to a lifetime depending on the primary cause which could be confirmed only after an endoscopy.
