Based on the facts that you have posted you seem to have recurrence of UTI and you have some left over antibiotics .

If the said antibiotic were effective in combating the infection you can take it However Since there is every possibility of change in bacterial flora I would suggest to get fresh urine test and culture done and take antibiotics as per culture report.
