Question: If an erection lasts for four hours because of, and not in the absence of, sexual arousal, does this still constitute priapism? If not, does it still carry the risk of damage leading to permanent erectile dysfunction? Is this damage guaranteed, or is it just a chance?

Based on the facts that you have posted you seem to have been facing problem of Priapism .

The extent of damage to erectile tissue entirely depends upon the type of priapism whether it is Ischemic priapism or Non ischemic priapism and frequency at which you get it .

You need to consult qualified Urologist for clinical evaluation when you get the Erection lasting for four hours or more .He may do following few tests to establish diagnosis as regards type of priapism .
1) Blood gas test (Penile Blood)
2) Color Doppler Ultrasound scanning of penis .

Non Ischemic priapism does not require any treatment except wait and watch policy.
