Question: I'm 24 years old women , weight 84 kg, height 172cms, and the result of TFT is as followsTotal T3 level is 114 ng/dl, Total T4 level is  10.0 µgram/dl, TSH level is is 0.63 µ IU/ml. I'm taking Thyronorm 125mg. Regular periods lag of 20 days. What should I do doctor?

Levels of your thyroid hormones (T3 and T4) are normal

Typically, normal Total T3 values range from 5 to 12 μg/dL).
Total T4 test in adults range from 100 to 200 ng/dL.

Reference range for TSH levels is between 0.4 and 4.9 mU/L.
Though your TSH levels are normal,but it does not indicate a thyronormalcy.

Even if the serum TSH is within the normal range as a result of thyroid replacement,the ideal target is to keep serum TSH levels of approximately 2-3 mIu/L.

Your TSH value is 0.63 µ IU/ml so your hypothyroidism is getting over corrected.
This becomes more important in the light of the fact that you are on high doses of Thyroxine (125 micro grams) and secondly the thyroxine supple mention has a long half life and serum TSH levels lag behind changes in serum thyroid levels.

Your Thyronorm dose needs to be revised.
Since how long are you on this dose of thyronorm?

Visit your endocrinologist for dose titration and get your thyroid profile repeated after 6 to 8 weeks of dose change.
The rationale behind the commonly used period of 6 to 8 weeks between dose adjustments is that changes in TSH response after dose change is quite slow.

Thyroid hormone plays an extensive role in body metabolism.
Menstrual irregularities are quite common in thyroid abnormality.
These get corrected when patient is euthyroid.

Thank you Doc . Is it possible to cut off Frisium 10 mg as I'm taking this medicine for last 9 years and is there any problems taking in pregnancy period?

HYPOTHYROIDISM AND ANTISEIZURE MEDICATION

Hi....
As with most patients with controlled seizures,you would also like to stop taking anti seizure medicines but this should only be done under medical supervision.

You are a candidate for tapering your medication if you had history of a few seizures prior to start of your medication and if,at present,you have a normal EEG,neurological examination and a brain scan.
But this is a rough estimation and at times,even people who meet these criteria can throw seizures on deescalating their regimes.

A decision about whether to stop or titrate seizure medicines is a prerogative of treating consultant and should only be made by him according to risk benefit ratio.
Usually doctors consider to taper down the drug dosage after a seizure-free period of 4 to 5 years.

Frisium is a drug belonging to Benzodiazepine group,used as an add-on drug for epilepsy and generalized anxiety disorder.
Anti seizure dose of it is between 5 mg and 40 mg a day, depending on body weight, efficacy and tolerability of the patient.
You are on 10 mg drug which is a minor dose,easy to taper and stop.

Clobazam (Frisium) being an adjuvant therapy in the treatment of refractory epilepsy is easy to titrate and discontinue but this should be done by your neurologist as doing so, may precipitate withdrawal effects and seizures.
clobazam is usually tapered off over 3 to 4 weeks in a gradual manner.

Generally Frisium is not prescribed to females of childbearing age,who are pregnant,who are planning for a pregnancy or breastfeeding mothers.
Talk to your doctor if you are planning to be pregnant.
