Question : I'm experiencing a very itchy, red and warm rash on the front of my groin area, in the fold of my skin. I am over weight so I have a belly overhang, that is where this rash has developed. It started a while ago and has cleared several times but always comes back. This time however the 1% hydrocortisone cream I have been using is not working. What is it and what can I do to clear it up? Please give me some ideas doctor

Sorry for the delay in writing back,
I have reviewed the image you have uploaded. This looks like intertrigo. This is a condition which arises following the constant friction between the adjoining skin folds. This can get complicated with the growth of yeast or bacteria.
I would suggest you to do the following:
- Always keep the area dry and clean.
- You can buy 1% hydrocosrtisone cream OTC along with zinc oxide paint, as these are available without a prescription. You can apply both of them once you have washed and dab dry the area. Do not apply Hydrocortisone for more than 5 days.
- Do not use any irritants like perfumed soap. Cleaning the area with water is enough.
- Try reducing weight.
- You can also apply SUDOCREM which is a thick based cream. You can apply it over the area once the steroid course is over. This acts as a barrier and prevents further collection of sweat in the area.
- You may also require an application of clotrimazole and beclomethasone ointment for a weeks time if there is any fungal infection. Since this requires prescription, you can choose to go for this if there is no considerable cure from the above steps.
