Question: Hello Doctor,

I am 31 years old...
My skin is sensitive to the sunlight...too much of exposure to sun would cause rashes with itching on the skin which is exposed to sunlight....
In the month of March I have got severe rashes and itching on my skin due to long time exposure to sunlight( face, neck, hands)...
I have consulted skin specialist near to me and he suggested me some medicines to use which included sunscreen lotion also...I have attached the prescription with the number 1 for ur reference...also doctor asked me to visit again after 5 days...
I am recovered very quickly with no complaints in 3-4 days and went to meet the doctor again after 5 days as suggested...This time doctor gave me another medicines to use ...I have attached this prescription with number 2 for ur reference...
Don't know what went wrong, after two days of using the new medicines in prescription number 2 , I have got black patches on the skin in the same place where I have suffered rashes 5 days back...I am unable to get rid of them now...
dark black patches on the forehead are bothering me more as I am unable to go out freely...
Picture of the dark patches on the neck has been attached for ur reference...
kindly suggest me a solution for this as I am unable to mingle with others...

Lichen planus pigmentosus and its management

Detailed Answer:

Hi,

I have gone through your complaints and reviewed the attached photographs.
It seems to be lichen planus pigmentosus.
If I was your treating doctor I would recommend you to apply topical steroid like mometasone cream on the affected areas once daily in the morning and tacrolimus 0.1 percent ointment on the affected areas at bedtime.
Also use a broad spectrum sunscreen on the entire face and neck area during the daytime.
Also you can take tablet isotretinoin 10 mg once daily at bedtime.
These all are prescription drugs and should be taken under the supervision of the doctor.

Thanks a lot doctor...I will follow your prescription...does they have any side effects? I have bp and thyroid. so plz confirm whether I can use them.

Also plz let me know for how many days I should use these medicines especially those tablets.. Thanks

Actually these medicines should be used under the supervision of the doctor.
Capsule isotretinoin requires monitoring of liver function test and lipid profile.
It is contraindicated during pregnancy and 3 months after stopping the medicine.
Even the topical medicines need to be taken under doctor’s advice.
