Is Epogen Safe To Take For Anemia With Kidney Injury?

Question: My question can Epogen be safely used to correct anemia in a patient with acute kidney injury without making their kidney injury worse? I know it's used in patients with CKD, but what about patients with acute component to their kidney injury. I was told if there is an AKI, giving them Epogen could worsen their kidney disease by stressing the kidney more.

Detailed Answer:
there is no problem if anaemic patient is given Epo (short for Epogen). Epo is hormone that is synthesized in kidneys normally and decreases in concentrations in blood when ever the kidneys are diseased.

The previous and logical thinking is that in AKI , the kidneys are temporarily shut down and they regain function very soon and all the functions if kidneys are restored immediately, also the Epo synthesis .

But several studies related to use of Epo in Icu with or with out renal shut didn't (regain their function) and associated with anemia, Epo supplementation reduced the number of transfusions but did not affect the outcome.

Hence the Epo can be given safely in pattients with AKI with out any major adverse effect.

However one should be aware of the adverse effects of Epo when it is being used in CKD such as worsening hypertension etc. it can produce seizure in those who are vulnerable.

Other wise no major complications known or contraindication associated with Epo

So what you are saying is any patient with AKI and anemia, even patients with AKI on CKD is safe. There is no chance of worsening kidney injury?
