Question: my fasting blood sugar is more (145) even after consuming 1000 mg Metamorfin after dinner as suggested by doctor . kindly suggest me what is to be done ?

1) Since 2011 i take Metamorfin, Recently consuming 80 mg Glizid As per doctor it is to control FBS.
2) Test report as follows
Blood urea 29mg/dl
Serum creatinine 1.0 mg/dl
Serum cholestrol 160 mg/dl
Serum triglycerides 202 mg/dl
Serum HDL 42 mg/dl
Serum LDL 83 mg/dl
SGPT 15 IU/L
SGOT : 21 IU/L
3) Present weight is 77 kg but varies between 76 to 79 kg
4) High BP, last checked is 131/ 88, takes 20mg OLMAY bp tablet in the morning. feels weakness. Suffering disturbed sleep.
5) On 31/09/2016 Number is 139 mg/dl.
6) eat 2 chapati and little rice for lunch and dinner. For breakfast chapati, idali, XXXXXXX Walking for a hour every day.

Thanks for following up with these details,

First of all, you need to get further specific set of investigations (a fresh FBS, PPBS-2 hr post prandial, HBA1C, Thyroid function test, CBC, Electrolytes, Vit B12 and Vit-D3, 12 lead ECG, Urine R.M to look for extent of involvement) followed by a clinical check-up by an Internal Medicine specialist (the cause for persistent weakness may not be related to Diabetes, however the glycemic control is important to determine here, which will be assessed after the HBA1C reports)..

2) Triglycerides a bit on the higher side, however rest of the parameters are grossly within limits, so better to start diet control even more strictly, it's advisable to get a thorough Diabetic Diet chart prepared under supervision of the Dietitian (you can simply request for a Diet chart in this portal only, if it's included in your package, do check for it and while requesting for it - mention your current body weight, height, food preferences etc.)

3) Incomplete data, parameters can't be calculated without height (which I've specifically asked for in the previous reply also)..

4) This range of BP is acceptable, considering your already on OLMAY, yes persistent weakness needs to be evaluated in details as already mentioned in the first column..

5) Wrong data, it's only 22/09/2016 in calendar year, please check the correct date..

6) Diet modification needed, your taking primarily Carbohydrate rich diet which has to reduced as per your daily requirement and betterment of blood sugar..yes continue the walking..

Avoid too many roti, rice, idli and other carbohydrate based foods, start including lot of fresh green leafy vegetables, salads, fresh fruits in your daily diet (also try to avoid fizzy carbonated beverages, packaged fruit juices, try to replace your daily wheat/flour to whole wheat/whole grain products).. do get the detailed Diet chart in this regard as already mentioned in column 2..

Coming directly to your present query, your BMI is around 28.6 which is in the overweight range, so I'd request you to go back to the previous reply and start implementing those dietary modifications as well as the initiation of comprehensive diabetic diet chart as already explained..

Yes, the FBS is still on the higher range as per the 31/08/16 report as per your description, however I couldn't find any attachments here..

However, you should start implementing all the advises as already mentioned, and even though HBA1C report is not here, still let me inform you that, it depicts the glycemic control of around 3-6 months only (means the 24/11/2014 report can only be used as a baseline data but a fresh repeated report is necessary to determine the present glycemic status)..

Don't worry, but follow all the advises as already mentioned and get the necessary investigations, clinical check-up done by an Internal Medicine specialist for a complete look in to it..
