Question: Tested negative on hiv1/2 p24 test after 8th day and again on 16th day post exposure. Also tested negative on hiv1 rna quantitative test on 8th day. Am i safe?

If you tested negative for RNA PCR on 8th day then you can remain relaxed. You are safe. But as per guidelines you should get HIV rapid antigen antibody duo done at 12 weeks and if that comes negative then no further testing is needed. But remain relaxed the chances that the test will come positive are very less, almost nil.

Thanks doctor, i got a quantitative hiv1 rna done, is that an incorrect test on 8th day? Also, i have aquired chlamydia, does that mean that i have a chance of hiv as well, the way chlamydia came in my body similarly hiv can also enter?

I am sorry for late reply. HIV RNA test is a very sensitive test and even at 7 days the test has sensitivity of about 95-99%.

So the chances that any test will come positive are almost nil. You can stay relaxed.

But I have mentioned the guidelines in my last answer. As per guidelines if Antigen Antibody Duo test comes negative at 12 weeks then no further testing is needed.

Otherwise remain relaxed, if test is negative at 8th day so the chances of coming positive in future are almost nil.

If you got Chalamydia that doesn't mean that you will get HIV. Chalamydia is also a STI but infection of this and HIV infection has no direct relationships. Take proper treatment of Chalamydia and you will recover from this. This doesn't mean that you will get HIV.

Thanks doctor. Following was my story to give u a context : i went to a massage parlor and got a nude body to body massage, she gave me full massage with her vagina exposed. I had not put condom. We did not have sex or oral sex. But i kissed her, not deep kissing and sucked her boobs. Then did some fingering. But my finger that was inserted had a small cut near the side of the nail, kind of what you get when you try to cut nails from teeth and peel the skin. The blood had come out for sometime in the morning. I got the massage in the evening. My penis was exposed to her vagina, and i think my tip of penis must have come in contact with her vaginal fluids. How concernibg is this.? Thougj i am tested negative on Hiv1rna and hiv1 combo test on 16th day. Few queries:
1. Do you think this cut could have exposed my bliid stream to hiv virus? How concerning is this.
2. I have read that hiv1 rna has high chance of giving error results. Can this be the case if done earlyAlso i have heard that the 4th generation duo test is conclusive on 28 days, should i get myself tested then as well. Though i am negative on 16th day.
2. Since the girl had chlamydia, i am not sure if she didn't have hiv
3. To treat chlamydia, i am taking azithromycin 500 mg daily with rantac 150 once a day. Is that okay? For how many days should i take it? Also, should this be taken with antacid, goigle tells otherwise.

Coming to your sexual activity. If we consider HIV virus then the virus can't survive for more than few seconds when exposed to air. So if your penis was dry and her vagina was dry and there were no exchange of fluids then the activity will come under minimal or no risk category as per guidelines. In that case the risk of exposure is almost nil.

You had slight finger cut in morning but in evening there was no bleeding and in 12 hours it must have healed. In that case even after fingering the risk is low.

You are tested negative with HIV RNA at 16 the day too so there is no need to worry. You can sit relaxed, but at 12 week for for HIV rapid and this will come negative too. This is as per guidelines.

HIV RNA has very high sensitivity and specificity at 8-10 days. You tested negative even at 16th day so the chances that any test will come positive in future are almost nil. So stop worrying for that.

Chalamydia and HIV are different things. If a professional sex worker has HIV they usually don't go for sex to prevent exposure to clients.

Yes the antibiotics are fine and you should take at least for a week. Take Ranitidine first and then antibiotic.

That's very reassuring sir. Just to correct the fact i got hiv1 rna test done on 8th day only and not on 16th day. On 8th day it came negatibe. On 16th day I got hiv1/2 p24 combo test which also came negative. I hope this does not chanhe anyhting.
Also, the cut which I had was not big, but when i went to massage, it was still red, like what you get when it is partially healed. . I hope this redness did not let hiv enter my body.

HIV RNA as I mentioned earlier too has sensitivity in range of 95-99% at 7-10 days. So that doesn't mater that you got tested at 8 days or 16th day.

HIV combo is most sensitive at around 4 weeks. But even on 16th day the test has sensitivity of around 95%. So remain relaxed.

Keep one thing in mind that, HIV is a very serious infection but its not easy to get HIV, unless you work hard to get infection. ;)

So the sexual activities which you performed were of minimal risk category. In future follow safe practices, use condoms during any sexual activity even oral sex. Avoid deep kissing, inserting fingers or active oral sex with commercial sex workers.

Hello doctor, one more thing .. In my std test, chlamydia igg came negative while iga came postive (2.9). Why is there a difference? Does this still mean that i have chlamydia, also, it was teated using blood sample and not urine. Is it still valid?

Chalamydia test is done using blood sample. Serum is tested for two kind of antibodies one is IgG and another is IgA.

Presence of positive IgA while negative IgG means that infection is still there or there is reactivation of infection.

So the test is valid. With successful treatment IgG will become positive and IgA will become negative. This means you had infection in past.

Talk to your doctor again and if needed repeat the antibiotic course.

Thank you doctor.. I have been taking azithromycin for last 5days now. Also, when should i next test for chlamydia again? Secondly since i had tested for hiv1/2 p24 combo on 16th day and it had come negative, in my opinion this means that there were no p24 traces, even if antigen and antibodies were not detected.. Then does that mean that my future tests will not show any antibodies.. As p24 is an indication of hiv virus in the body?

Yes p24 is an indicator for HIV virus infection and in most individuals with infection the antigen is formed in around 10-12 days of exposure. Though I am not sure about sensitivity of HIV 1/2 p24 combo on 16th day but remain assured that if the test is negative at 16th day then there are very remote chances that any HIV test will come positive in future. As mentioned earlier that final test should be done at end of 12 weeks. In most cases if combo test comes negative at 4 weeks then these could be taken as conclusive (sensitivity of 95-99%) while at 6 weeks the sensitivity is around >99%.

But your PCR RNA test at 8 days were negative and this is a very confirmatory test. So stop worrying about HIV. You are safe dear friend. You were engaged in no direct sexual activity so stop even worrying about HIV.

For Chalamydia complete the course of Azithromycin and then get repeat test after 4 weeks. This will come negative for IgA.

Now keep one thing in mind that you don't have to go for repeat HIV test for at least 4 more weeks. Get HIV combo test at 6 weeks of exposure and this will definitely come negative.

I was worried because on internet lot of websites were saying that rna tests give lot of false results and hence are not reliable, also i got my combo test done at 16th week, which again dont give 100 percent conclusion. So was worried.

If we consider the risk behaviours of HIV transmission then risk of transmission by

- Oral intercourse (receptive or insertive) minimal or low
- Vaginal intercourse insertive 4 out of 10000 for receptive 8 per 10000
- Anal intercourse insertive 11 per 10000 while for receptive 138 per 10000
- risk is more with needle sharing and very high in blood transfusion.

The risk is negligible in other form of sexual activities. Negligible means zero or rare cases occurred by this mode of transmission.

So by the sexual activities you performed the risk is practically ZERO.

There was no need to get tests done. But yes to relieve anxiety tests are needed. You tested RNA PCR negative at 8 days (while those who contract HIV usually test positive at this time) and Combo at 16th day (though not very sensitive at this time but a lot of individuals test positive at this time). You came negative in both, so why are you anxious?

And yes RNA tests are reliable.

First of all you are not sure about the status of lady at massage parlour. Why are you thinking that she was positive? In most cases girls who are in massage parlours or are commercial sex workers are trained properly to prevent HIV transmission. So most likely that lady was negative.

Even if we assume in remote chances she was positive even then the sexual activity you performed carries negligible risk or zero risk.

The tests are also negative.

So stop remaining worried. You are safe, just because of anxiety you are stressed.

I don't see any rash here. The itching and dryness is probably due to antibiotic therapy. Antibiotic therapy results in itching and this itching could cause redness because of irritation. There is nothing to worry for that.

HIV real time PCR is a quantitative test. The test measures RNA in log copies. The range is 1.3-7 log copies/ml (20-0000 copies/ml). If there is no detection of RNA in blood sample then this will be reported as RNA <20 copies/ml.

"THIS MEANS NO RNA WAS DETECTED IN BLOOD SAMPLE AT TIME OF TEST"

So you can remain relaxed. This doesn't mean that your blood had HIV RNA copies.

No dear hyperacidity has nothing to do with HIV symptoms. HIV diagnosis is not made on the basis of symptoms. The symptoms you are having in past 4 weeks are more due to anxiety and not related to any serious issue.

HIV 1/2 p24 combo test at 4 weeks has very high sensitivity. If we go by records then only in rare cases a person can turn positive later on if the test is negative at 4 weeks. p24 antigen is formed at around 2-4 weeks and if your blood had any of infection then the test would have detected it.

So the chance that you will test positive in future is NIL.

Based upon the sexual activity you performed and the results of tests RNA PCR at 8th day and Combo at 4 weeks, I don't think there is need to go for more tests. This will only cost you money. YOU ARE NEGATIVE AND SAFE.

The sensitivity is around more than 99% at 4 weeks. The test at 4 weeks is considered as conclusive.
