Brief Answer:
No need for any further test.

Detailed Answer:
Dear XXXX,

Welcome to HCM.

You had exposure in August 2014 and HIV Ag/Ab combo test is negative after nearly 2 years then it is 100% conclusive because the window period for HIV also varies from 15 days to 3 months so calm down, forget about the episode and move ahead in life. No need to go for any further test for HIV. For any further query feel free to ask me any time.

Thanks for the reply sir
I underwent a master health check up on august 2016 they check the report and said everything is ok but my creatinine level is elevated 1.4 but serum urea remains normal 19 and the reference value is in the range of 0.3 to 1.3 they said nothing to worry drink more water is enough no need for medication but after 10 days i again tested at a lab it goes 1.7 and urea remains 17 so underwent treatment the doctor prescribed nefrozon , neubon and esomac 40 whether this medication will affect the accuracy of my hiv combo test when i took the test for hiv i also took creatinine test in the same lab it remain elevated 1.4 but my hiv duo test remains non reactive 0.08

Sir i have certain doubt in my health check up total protein is about 7.9 and the reference range is 6 to 8 is there any possibility for reduction in accuracy of hiv duo test due to high protein content and maximum how days require to produce detectable level of antibody after past exposure . Taking any medication can delay the antibody production if such what are such medication . What about the accuracy of hiv duo test after two years

1. What are the common strains in XXXXXXX that is in group m, n, o and p of hiv 1 plz reply me first and foremost briefly for that question whether everysubtypes produce several pattern that cannot be detected or i read in forum subtype c is most prevalent strains in XXXXXXX and there is any increase in number of case regarding group o and hiv 2 stats
2.whether i tested with hiv ag/ab test (cmia) method has detected all the subtypes of hiv 1 and 2 or not
3. I have tested for hiv for the first time after 26 months of exposure is there any possibility of me to still in the window period or what is the maximum time period for a person to seroconvert despite of medication or any other condition
4. From august 2016 i have several times facing chapped lips . If herpes sores and chapped lips are same ?
5. Whether i need to test for herpes virus ? Is there any possibility of getting infected .
Hope again you reply me as in the order i asked detailed waiting for your reply
doctor
Answered by Dr. Sunil Jangid (5 hours later)
Brief Answer:
Sub-type C is most common in XXXXXXX

Detailed Answer:
Dear XXXX,

Welcome to HCM once again.

The most common strain of HIV in XXXXXXX is C and HIV 2 is very uncommon in XXXXXXX If you tested with HIV Ab/Ag then it will detect HIV 1. If you are negative after 26 months of exposure then there is no possibility of window period even with any of medication or other condition so allay the fear or anxiety of HIV.

Herpes sores and chapped lips both are two different conditions and I don't think if you are infected with herpes but still to relieve the stress or tension you can go for herpes testing and the result will surely come in your favor. Be relaxed, forget about the episode and move ahead in life happily with your family.

3) how accurate hiv combo test after more than two years whether it is accurate for lifetime after the window period or not ?
4) i have screened for both hep b and hep c both are negative but i have certain doubts whats they are mentioning in the hbsag screening that is please not the reference interval ?
5) whether my herpes igg antibody test is reliable and conclusive or not ?
6) how long the p24 antigen can be detected in the blood the p24 antigen starts decreasing once the antibody for hiv starts developing this is true or not ?

Brief Answer:
Antibodies will remain for lifetime.

Detailed Answer:
Dear XXXX,

Welcome once again to HCM.

Once you get the infection antibodies will remain in your blood for life time

You need not to go for repeat testing because there is no possibility of any late sero-conversion.

HIV combo test is 100% accurate after window period. False positive is not possible after such long period so don't worry.

Herpes Igg test is also reliable and it is routine protocol of laboratory to mention about reference interval.

P24 antigen is also detectable any time after infection and it will not decrease once antibody production started so just calm down, forget about the episode and move ahead in life happily.

Brief Answer:
Any value below one is considered negative.

Detailed Answer:
Dear XXXX,

Welcome back and thanks for follow-up.

I have seen all the reports so you need not to doubt on the accuracy of test result as any value below 1 for HIV testing is considered non-reactive. Despite any symptom also the result is ddefinitely accurate after window period so don't get feared or anxious.

There is no any peculiar feature of rash associated with HIV but the details you had mentioned is most likely pointing towards allergic rash. Consult your physician to isolate the cause for pre-determined immune deficiency and follow expert treatment.

There is no such causes for late sero-conversion it is just the concept of testing after 12 weeks which is required if patient is on PEP or chemotherapy so calm down, forget about the exposure and move on in life. For any help feel free to ask me again.

Answered by Dr. Sunil Jangid (11 minutes later)
Brief Answer:
The best solution is to get tested.

Detailed Answer:
Dear XXXX,

Welcome back once again.

Pre-existing immune deficiency will affect antibody production that is why testing after 12 weeks also required in such cases and to diagnose deficiency you have to for blood tests as per doctor advice.

There is no such difference between allergic or HIV rash so the best way to confirm is HIV testing after any exposure otherwise dermatologist will diagnos whether it is allergic or other skin conditions. If still you have any doubt feel free to ask me again.

What are the condition for late seroconversion?
1) taking pep
2) chemotherapy treatment
3) preexisting immunodeficiency these are the reason for late seroconversion
What is the maximum time for seroconversion ?
If they have undergone above conditions?
I didn't take any pep and chemotherapy treatment
My only doubt is about pre existing immunodeficiency whether i have pre existing immunodeficiency or not
If i have it is it possible for me to not developed antibodies to hiv after more than 27 months
Last things is plz prescribe which test to be done to check pre existing immunodeficiency reply me in detail and prescribe me the test must sir
doctor
Answered by Dr. Sunil Jangid (14 hours later)
Brief Answer:
It is not more than 3 months.

Detailed Answer:
Dear XXXX,

Welcome to HCM once again.

The maximum time for seroconversion is not more than 3 months and even when you have preexisting immune deficiency then also antibody production will not delay upto 27 months so don't get worried.

You have to go for blood test after consulting your physician to rule out immune deficiency like ANA. For any further help feel free to ask me. Be relaxed, forget the episode and move ahead in life happily.

3) whether did you see someone with preexisting immunodeficiency who can seroconvert after 2 years?
4) did you see anyone with preexisting immunodeficiency? In how many days they can seroconvert after exposure?
5) what is meant by ana immunodeficiency disorder?

Brief Answer:
You can go for RNA PCR for your satisfaction.

Detailed Answer:
Dear XXXX,

Welcome back and thanks for follow-up.

As such you don't need any further test for HIV but for your mental satisfaction go for RNA PCR. Also it is not possible that someone get sero converted after two years even with immune deficiency so be relaxed and forget about the past incidence.

ANA stands for anti nuclear antibody test and also other tests are available to diagnose pre-existing immune deficiency for which you have to consult your physician and follow the expert advice. If still you have any query feel free to ask me any time.

Hai sir , today morning i went to the lab and ask for hiv rna test they asking whether you have positive antibody screening? I said no they said then you need not go for testing rna it is only for the people who have positive antibody screening then only suggest hiv rna test to check viral load and drug assessments you need not to waste your money . They said better you go for western blot or otherwise take an antibody test after 3 months i said it already more than 2 years they said then you don't need to worry i have certain question to ask
1)Whether if i have low copies of viral load for example if 10 then our body will produce detectable amount of antibody after two years of possible exposure or not
2) they suggest me to go for western blot whether western blot is reliable and it completely give a picture to relieve my stress
Please need a detailed answer for the questions above sir please i beg u whether if u have undetectable viral load and your body porduce detectable level of antibody or not please after two years of exposure

It is not possible that if you have low copies of viral load then your body will produce antibodies after 2 years of infection and not only western blot but the antibody test what you have done also gives same accuracy and reliablity after 3months as truely said by laboratory people.
