Question: Does trammadol150mg dily with Mobic,Dothep25mg,Codien 30mg Panamax 500mgFruesimide used when needed !Valium 2mg when needed other above tablets each day plus I was on Valproate 200mg tabs x5 per day until last Feb 2013 for about 8 yrs ,can it cause Cardiac arrest & low form of Cardiomyopathy, looking forward to yours & colleagues answer please XXXXXXX XXXXXXX

Brief Answer: NSAID can increase the risk. Detailed Answer: Hi XXXXXXX XXXXXXX I went through your query and understood your concerns. Mobic (meloxicam) and Panamax(paracetamol) are a nonsteroidal anti-inflammatory drug (NSAID) that may rarely increase the risk of a heart attack or stroke. The risk is greater if you have heart disease or increased risk for heart disease (smoking, family history of heart disease, or conditions such as high blood pressure or diabetes), or with longer use. Even Valproate have been shown to have association with heart attack but the incidence is more in initial month of use and more in males > 60 years. But there is no direct association with the drugs. The common cause for Cardiac arrest & low form of Cardiomyopathy is sedentary life style, uncontrolled BP or diabetes, over weight, smoking and alcohol history and genetic factors. The reason I am asking about the medications is that when I had my cardiac arrest the drs in Adelaide said that it was caused by medication that affected my qt & qtc& Cymbalta is one of the tabs but they have left me on them at 30 mg then a ECG then upped them 2 60 mg with a ECG each 2 weeks then upped them 2 90 mg repeat ecgeach 2 weeks last week qt&qtc were high now back down 2 60mg Cymbalta still repeat 2 weekly ECG is it wise 2 keep on them .could you please pass this email & other email onto a heart specialist as I need answer as I have chronic back pain due 2 work injuries & I am under a pain management specialist for pain& he seems not 2 be sure now what I can be given & I told him I'm shit scared 2 take medications in case it causes another cardiac arrest as I was told by drs only 1 in 50 people survive this kind of arrest

Brief Answer: Be relaxed. QT prolongation is very rare Detailed Answer: Hi, QT prolongation can occur due to many causes like genetic cause, your illness or due to medication. Cymbalta is not known to cause QT prolongation but since it is a SSRI group a possible interaction between this and thiazide diuretics is associated with QT prolongation. Otherwise, if you are not on diuretic then no need to worry. Even the QT prolongation is dose related and hence you doctor has adjusted the correct dose. Disease condition like cardiac problem and hypothyroidism can also lead to QT prolongation. For analgesic action (Pain killer) you can use any NSAID and even opioid other than methadone are not know for QT prolongation. So your pain management specialist can use the above said drugs.
