Is Morphine Effective In Treatment For Throat Cancer?

Question: Sir
my father is having throat cancer. Age 70. He is taking only juices & liquid food only. Chemo & radio therapy done 15 months before. As per the dr. Instruction he is taking morphine tablet 10 mg x 3. Is there any fruitful remeady?

1. This depends upon the extent of the throat cancer and whether it was involving the upper food pipe (esophagus).

2. A PET Scan can now reveal whether the cancer has been eradicated or is still active.

3. A videolaryngoscopy and an flexible endoscopy along with swallowing studies can help understand why he is only able to take liquid food only.

4. Post radiotherapy and chemotherapy, damage to the normal tissues of the throat and neck can cause dryness and pain. Does he respond to injectable painkillers? Is his pain burning type or deep aching type?

5. In some patients dilatation of the esophagus (food pipe) can help.

6. If the cancer is still active, only palliative treatment is possible. Kindly fill in the above details, if possible.

Sir, his pain is deep aching type. CT scan dt 24.05.13 shows the following:-" a large irregular soft tissue mass of 6.5x5.1x4.2cm size in oropharynx posteriorly & laterally on the left side. Involving left posterior, lateral walls, tonsillar goes a, posterior third & base of tongue & with left cervical lymphadenopathy. No evidence of bony revisions or supra clavicular lymphadenopathy is seen. "
After that chemo & radio therapy given.now during pain in ear & neck of left side he is taking the only medicine of morphine tablet 10mg. Please suggest any medicine. Thankfully. Saravanakumar.

Thank you for writing back.

1. The large irregular soft tissue mass in the oropharynx needs to be re-evaluated by an endoscopy under general anesthesia.

2. Multiple biopsies will be required. FNAC (Fine Needle Aspiration Cytology) of the cervical lymph nodes needs to be done. PET Scan will be useful.

3. Instead of morphine, consult an anesthetist specializing in pain management. A Fentanyl patch may be a good alternative.
