Question: Hello Dr XXXXXXX , Iam a 35year old woman and this is my 2nd pregnancy. My elder child was born 8 years ago through a normal delivery My LMP: 16th Jan2014 I have currently undergone 3 sonogrophies 1) 5weeks days: 2) 7weeks 4 days 3) 11weeks 4 days (NT SCAN) My NT Scan results are as follows: Single live intrauterine pregnancy of 11weeks 0 days. Nuchal translucency is within normal limits. Nasal bone is present. Follow up USG to rule out any structural anomaly is suggested. My first sonography detected multiple fibroids. Fibroids are seen along anterior and posterior wall largest measuring 3*2.3cms in fundic region. All other tests are normal. I have been prescribed the following medications: 1)Folic acid-once a day 2)Susten 200-twice a day..taken vaginally 3)tab doxinate*3 before meals 4)Tab Li 52*after food for 15days 5)Im Neurobiom alternate day for 6 days My queries are as follows: 1)My pre pregnancy weight was 73.5kgs.I was ok till 5 odd weeks I was put on susten (apparently for my fibroids). Some days after that, I have started severe bouts of nausea and vomitting Currently Iam 67 kgs. Last week my doc prescribed Neurobion and Tab Liv From what i have researched on the Internet, Susten 200 is preogestrone. Do I need to take this even when my fibroids are so small? I feel that Susten is causing me so much of nausea. My Doc says that its necessary to prevent miscarriage. Today for instance just till afternoon, I have thrown up 3 times.I have absolutely no appetite and have lost count of the no of times I throw up. doxinate does not help at all. I have this very bad taste in my mouth. Pls advise if I should discontinue the use of Susten200. I must mention here that I also have severe nausea during my first pregnancy. 2)I have nearly completed 3 mths of pregnancy and have already undergone 3 sonographies. Is it advisable to have so many ? I go to a very big hospital and they say the pros of a sonography far outweigh the cons. Pls advise. 3)Does the Neurobion inj help? I noticed that after I had the injection today, I immediately threw up. Pls advise 4) Does my NT Scan look ok? Thank You

Brief Answer: EXPLAINED BELOW Detailed Answer: Hello XXXXXXX Thanks for trusting me with your health concern yet again. Congratulations on your pregnancy. Let me answer your queries one by one - 1. Can you please upload the said ultrasound report that mentions fibroids ? That will help me to comment whether progesterone support is required. In general, fibroids during pregnancy represent an increased risk of preterm labour . Hence, progesterone therapy is definitely recommended to prevent that risk. You are prone to nausea and vomiting, and Susten is aggravating that. I have an excellent suggestion to make. Stop taking the tablets orally, even vaginally, and take local gels. Susten and Crinone progesterone gels are available with an easy applicator for vaginal gel use. That will serve the purpose of progesterone, and eliminate the miserable side effect of nausea and vomiting. Some women are prone to nausea more than others, the good part is that you have crossed 11 weeks, and from this point on, it will just get better. Just have saltine crackers early in the morning, before brushing your teeth even. Have small frequent meals all through the day instead of 2 -3 big ones. Let your main caloric intake consist of fluids - water, nimbu pani, buttermilk, coconut water etc. May I know which brand of Doxinate are you taking ? Personally I would suggest Sinate -OD . It has to be taken just once a day, and is wonderful at mitigating nausea, vomiting and unpleasant sensations that you are experiencing. Take candied ginger too all through the day. 2. There are no side effects of a sonogram, no radiation involved. You can have multiple ones, as long as they are indicated. 3. Neurobion will help in relieving nausea, vomiting and the weight loss. It might be a coincidence that you vomited just after the shot. 4. Just the typed version of the NT scan is not enough for my comments. Please upload the NT scan report too, whilst you upload the earlier ultrasound. 5. Many women experience weight loss during the first trimester, and dont worry, as long as the baby is fine. You would pick up weight once 14 weeks are crossed. All the best Looking forward to hearing from you. Take care.

Thank you for your reply 1)I have uploaded pics of the USG where they have shown the Fibroids. the rest of the pics are of the subsequent sonographies. If I need to buy susten in a gel formal, what is the drug name I need to give at the pharmacy? I truly hope that it works. 2)The Brand name of Dixonate that I use is Svizera, consisting of Doxylamine Succinate 10mg and Pyridoxine hydrochrolride 10mg 3)Can I directly start off with Sinate-OD? When do I need to take it? 4)My blood group is A-ive. My son is also of the same blood group. I would need to take the Anti D Injection only if my second baby is of a Positive blood group right? thanks

Brief Answer: AS BELOW Detailed Answer: Hello again. 1. I would wish to see the exact verbatim ultrasound scan reports, not the images, the images are not informative unless I real time scanned you. There is no need to go through the cumbersome task of uploading the images, just upload the scan reports. 2. Just ask for Susten / Crinone gel. 3. You can directly start off with Sinate - OD, once a day, best taken early in the morning , you can even take it twice a day for more relief if one tablet does not suffice. There is no need to take folic acid separately, as it is included in Sinate - OD. 4. Does your husband have a positive blood group ? Yes, Anti - D needs to be taken only if your baby is also having Rh positive blood group. Take care .

Brief Answer: AS BELOW Detailed Answer: Hi again I have gone through the reports that you sent. The NT scan is perfectly normal, nothing to worry about. Also, yes, with multiple uterine fibroids, please DO take progesterone right through the pregnancy, as you are at risk of preterm uterine contractions. Try the gel, am sure you will tolerate it better than the tablets, also with Sinate - OD, you will soon feel better Take care.
