Is PCR RNA Test Done Post 11 Days After Exposure Conclusive?

Question: Hi Doctor. I had two sexual exposures. One on February 15 and the other on April 6. I got tested for HIV. I did a PCR RNA on 23rd April along with HIV Ag and An on the same day. The testing was more than 60 days after the first exposure and 17 days after the second exposure. The first exposure was that I have rubbed my penis between the thighs of a massage girl and the second exposure was I rubbed my penis on the nipples of another massage girl and sucked her nipples. I didn't notice any milk or other secretions from her nipple. I have done the following tests:

1) HIV PCR RNA on 21st February -7 th day after first exposure.
2) HIV duo - 21st February - 7th day after the first exposure
3) HIV Duo - 30th day after the first exposure.

4) HIV PCR RNA - April 13th - 7th day after second exposure and 58th day after first exposure
5) HIV Duo- April 13th - 7th day after second exposure and 58th day after first exposure
6) HIV PCR RNA - April 18th - 12th day after second exposure and 63rd day after first exposure
7) HIV Duo- April 13th - 12th day after second exposure and 63rd day after first exposure
8) HIV PCR RNA - April 23rd - 17th day after second exposure and 68th day after first exposure
9) HIV Duo- April 23rd - 17th day after second exposure and 68th day after first exposure

All the test results were negative. I checked with the doctor and the PCR RNA is sensitive to 20 copies and he has told that HIV is out of question from both the exposures.

I am feeling very anxious and want to get your opinion if these tests are sufficient. I keep reading that 3 month test is conclusive and some people also say that PCR RNA is conclusive after 11 days. Can you please let me know if there is a risk in rubbing penis on the nipples and sucking the girls nipples . I checked with her and she told that she is not lactating and no other secretion from the nipples.

Can you please also let me know if these test results are conclusive.

Welcome and thanks for posting your query to the forum.

I can understand your concern for the two exposures and interpretation of results of HIV testing.

I first of all want to reassure you that your kind of exposure seldom results in HIV and your tests are almost conclusive for the same, so just relax first.

Let me simplify the things and give you medical and scientific facts behind my firm declaration-

Let you know about HIV infection: In any type of exposure with a HIV positive person the HIV viruses gets entered into the body and divides in innumerous numbers(millions of viruses per ml).

After a period of 2-6 week there is mounting of antibodies against the viruses. These antibodies can take at the maximum of 8-10 weeks of the period to get expressed.

There fore the infection can be analyzed by two ways -

-A): By HIV virus detection: done by RNA- PCR. It tells us the number of copies of virus present in per ml of blood. By the latest of methods the viruses can be detected within 2 weeks of period.

B): By HIV antibody Detection: As told earlier once the antibodies appear maximum of 8-10 weeks then these antibodies to HIV virus persist throughout life.

The protocol of the antibody tests done for HIV is as follows: HIV antibody test at 3 months. Antibodies once formed to HIV can never disappear. So if a person has HIV infection, 3 months of period is the time when we say to be fully sure of whether HIV is present or not.

Now correlating all the facts and interpreting with your situation:

In my clinical experience with your kind of exposure I have never seen any positive case. The act of rubbing the penis between the thighs of a massage girl and the second exposure of rubbing penis on the nipples of another massage girl and sucking nipples are not the usual mode of exposure to HIV virus.

A RNA PCR test is of best help in all the anxious cases who do not want to wait till even till 90 days. Your test of PCR is conclusive, so relax your mind from HIV.

So i understand that the tests i have done on the 17th day after the second exposure is final and 100% conclusive and that there is no need of any further tests.
