Question: I AM DIOGANISED WITH CHRONIC DUODENAL ULCER.MY AGE 52 MALE,HEIGHT 177CM,WEIGHT 92 KG,WORKING AS ENGINEER ,POWER PLANT,NO REGULAR EXERCISE,NO ALCHOHAL, VEGETARIAN, OBESE, SWEET INTAKE ABOVE AVERAGE.CONDITIONS PRESENT FOR LAST ONE MONTH. /LIVER ENLARGED IN SIZE 17.3 CMS AND SHOWS DIFFUSE FATTY CHANGES WITH NO EVIDENCE OF FOCAL PATHOLOGY.USG IMPRESSION IS 1.HEPATOMEGALY WITH DIFFUSE FATTY CCHANGES IN LIVER
2 GRADE 1 PROSTATOMEGALY
DOCTOR ADVISED EXER DOCTORCISE AND RANITIDINE FOR 8 WEEKS WITH VARYING DOSAGE.

I have gone through your query and understand your concerns. Your doctor has guided you in the right direction. You need to take ranitidine regularly twice a day. You also need to take plenty of green leafy vegetables in diet. The fatty deposition in liver is often associated with raised blood sugar levels and I would insist you to get a fasting blood sugar as well as HbA1c level checked. If these are insignificant, you would not really need to be bothered about the fatty change.

About the duodenal ulcer, stress reduction is a major step that will help you. Yoga and meditation would help. Let me know the improvement that occurs with ranitidine. Further plans can be suitably made thereafter.

Be sure to take small meals but at smaller intervals. Avoid spicy food.

my use of ranitidine for past one and half months reduced my burpings, no pain in stomach but i feel my liver is bloated condition.
My Glucose serum/plasma(fasting)(GOD/POD) is 101 mg/dl and Urine glucose (strip method)(fasting) is nil.

Dear doctor
i am uploading my blood test done recently as per your advice.
i do exercise daily and reduced my sweet intake to . nil level .my earlier weight before one month was 92.6 and now it is around 91.5 avg.
still my stomach bloating is not reduced,and i leave lot of gasesand i have frequent urination urge after eating. my ulcer is2 months old since diagonised,i rarely have pain.ranitidine dosage is 1 per alternate day as adviced by my doctor.my question is
1.when will my bloating of stomach will come down.is there any other criticality to fear( i feel not excessively fatigue,but not also very active).i am worried and feel depressed.i have some times severe itching on my backand hands occasionally.My Hba1c is 5.8%.my urine is some times pale yellow but generally white by visual.My stool colour is not fully brown but pale yelloe or whitish yellow.Do i need to go further special test if any ?i am enclosing blood report for your kind perusal

Thanks again for writing back. Sorry for the inadvertent delay in replying.

I have gone through the attached reports. Our gastrointestinal tract has a forward propulsive movement (peristalsis) that moves the food material forward as it is digested, absorbed and finally expelled as feces. If somehow this movement gets hampered, food along with the digestive juices would stagnate, stomach acid will reflux into the food pipe and on the other hand the bowel habits would not be proper. Degradation of undigested food in the intestines by the intestinal bacteria would lead to the excess gas formation.

In order to relieve yourself, you need to do the following:
* Avoid spicy food. Take small but frequent meals. Avoid long gaps between meals.
* Avoid alcohol intake (if it a part of your habit).
* Take ranitidine or omeprazole once daily before breakfast.
* Take plenty of green leafy vegetables as well as other sources of dietary fibers. Take fiber supplements like isabgol/psyllium husk or senna. I would insist you to increase your dietary fiber intake.
* Regular morning walk and exercise to keep the bowel movements proper.
* If I had a patient like you I would perhaps have suggested Prodep 20 mg tablet once daily for at least 2 months (it may take time for the action to set in).
* Take curd or yogurt on daily basis. It contains beneficial bacteria and should help.

With proper interventions things should be back to normal soon. It is hard to provide a definite time frame.

The vitamin D level is low and requires supplementation. The HbA1c level is fine and you need not worry about it.

can i take cod liver oil tablets like seven seas brand to boost up vit-D.(i am veg, i cant eat direct fish or non veg)and also DAB(tender coconut water in morning) along with break fast.will it not interfere with ulcer?
my bowl movement is regular mostly once in morning,occasionally twice.
triglycerides level as indicated in my recent report also worrying.( i am non alcoholic).

Yes, you can take Cod liver oil capsules. It will help in boosting Vitamin A and Vitamin D levels. But before that I would have preferred Inj. Arachitol 3 lakh units (Vitamin D) intramuscular one such each month for three months. For your ulcer, eating light and bland diet is advisable. Be sure not to allow long gap between meals. Triglyceride levels are dependent on the food habits. Dietary restriction is all that is needed, as of now.
