Question: Hi Doctor,
im 36 year old female. Hypothyroid since 11 years. Currently im having extreme anxiety and palpitations at night. My heart beats goes to 130. BP is normal around 110/79. Im unable to lie down and sleep. Iam on anxiety medication since 5 years. Duloit 30 mg and take PETRIL sos.

2 weeks back felt choking and went to cardio. He did my ECG and sugar test which was normal. Recently I had bacd throat and allergy came in IGE blood test. I had taken several antibtiotics which were prescribed to me. I mean im scared of nights now as soon as i lie down heart starts pounding. I feel scared as if something will happen to me.
Please help as Iam unable to sleep and very much scared.

I had anxiety since 2010 and suffering from panic attacks can thyroid cause that?

Medicines Iam on since 5-6 years
-thyronorm 100 mcg, glycomet 1000, Duloit 30 (for anxiety)
medicines prescibed by endo
-thyrowel, pantocid L, ventoz,
my current TSH level is 0.8, HB 10.
IGE serum- 345
Iam really very scared..Please help..can I get cured?

Hello XXXX,

Thanks for writing to us. I can understand your concern.

You are likely suffering from panic disorder.

Not hypothyroid but hyperthyroid can cause panic attack. You have mentioned TSH 0.8 and that is normal. I still suggest going for T3,T4 and TSH to check that 100 mcg thyronorm is appropriate dose.

Petril contains clonazepam and it is good anti anxiety drug from benzodiazepine group. You are taking it sos. I recommend taking it thrice a day for a while and later on taper it gradually.

Duloit likely contains duloxetine. it is used for depression and somatic pain complaints. I never used it in panic disorder. Rather I will prefer paroxetine or mirtazapine. Please ask your doctor about the same. If you have not consulted psychiatrist, please consult psychiatrist once.

Best treatment of any panic disorder includes both medicines and cognitive behavior therapy (CBT).

In mild cases only CBT is sufficient. In moderate to severe cases initially medicines and then CBT is added. And later on medicines stopped gradually.
If medicines alone are used chances of relapse are more after stopping the medicines.

In medicines SSRI group of drugs (fluoxetine, paroxetine, sertraline, escitalopram) are main stay of the treatment. I prefer paroxetine. Benzodiazepines group of drugs (clonazeapm, lorazepam or etizolam) are used initially for short duration for fast symptomatic relief.

High IGE suggests some allergic problem. It is not so much hogh. Taking anti allergic like levocterizine or fexofenadine for a while will help. Repeat IGe testing after 3 weeks.

Iam feeling much releived now. Can you please tell me if this has something to do with heart? Duloit was given to me because I felt strange sensation in my left had and leg..there is a nerve which vibrates and gives me a tingling feeling whenever im sitting for long.. I mean i dont know but i feel wierd at times.

Iam from XXXXXXX where are you based? Can i see you? if not then can you recommend some good psychiatrist for CBT?

Your ECG is normal and you have been checked by doctor so there is nothing to with heart. In most of the patients with panic disorder this type of apprehension is common.

Yes for strange sensation duloxetine may help. I recommend serum vitamin B12 level for that if not done yet.
