Is A CMIA Test Done At 19 Weeks Conclusive?

Question: Dr. XXXXXXX

I would kindly ask your opinion about the need of additional testing regarding my specific exposure…. Pls find below my potential exposure, symptoms, undergone tests and, finally, my questions...

Potential exposure:
- Deep kissing/ Fingering/ Unprotected oral (receiver)/ Unprotected vaginal with csw.

Following symptoms:
- About 10 Days: Involuntary muscle contraction (mainly in lower legs/ cramps) + diarrhea
- About 02 weeks : White tong/ dry mouth/ dry skin + diarrhea
- About 03 weeks: Strange rash in my legs/ arms/ trunk/ neck (petechia like) + involuntary muscle contraction (mainly in lower legs) + night sweats for 03/04 days
- About 35 Days: Sore throat + nasal congestion + joint pain + involuntary muscle contraction (whole body)
- About 07 weeks: -> went through medication for sinus (azithromycin + 03 x 60mg prednisone/ 03 days + nasonex nasal spray for 30 days)... no fever...
- Up to now (19+ weeks): + white tong/ dry mouth/ dry skin over all body + some involuntary muscle contraction + loose stools.

Undergone tests:
- 14+, 25+ days: Undetectable HIV-1 VL (cobas taqman roche 2.0)
- 04, 06, 08, 12 , 16, 19 Weeks: Negative HIV 4th gen (roche combi pt)
- 19 weeks: Negative Roche Multiplex Test (hiv 1&2 rna, hbv, hbc).. HIV-2 RNA LOD (95%) = 2,2 copies/ ml... (100%) = 12,2 copies/ ml.

Questions:
- Can I consider the results fully conclusive about HIV-2 (negative HIV-2 RNA and 4th gen CMIA tests at 19 weeks)?
- Would you recommend additional testing for me?

Congratulations. You are free from both HIV 1 and 2, HCV and HBV. You need no further tests. All your symptoms are not related to HIV. They are independent and might be due some other common infection.
White tongue and diarrhea can occur to anybody without a HIV infection.
Your physician would take care of all your symptoms and forget about HIV and about the high risk exposure.

Welcome back.

Normally HIV antibody tests become conclusive around 8-12 weeks on wards. You had prednisolone tablets just for 3 days. Nasal spray has very less influence on immune system. You had your test negative even after 19 weeks.
So your result is conclusive and no further test is necessary.

Dr. XXXXXXX in the case of HIV-2, would it possible to have a viral load below 12,2 cp/ ml and undetectable ab after 19 weeks?

Can I be 100% sure that I'm negative?

Welcome back.

Viral load below 12.2 copies along with undetectable antibodies after 19 weeks definitely ruled out the possibility of HIV 2 also. Results are almost conclusively up to 100%. So you need not worry at all.

Dr. XXXXXXX would an increase in the negative ab test index mean something? My last one was 0.21, but the previous one was 0.16... Cutoff = 1.00.... Tks again.
