Question : I just switch form coumidan (warfarin 7.5) to xarelto 20mg. My nri was 1.2 after 7 days is this a safe level?

Xarelto at 20 mg is a reliable dose with adequate anticoagulation for most individuals. We do not follow the INR with this medication as we do with coumadin as the Xarelto does not affect the INR in the same way that the coumadin does.
As long as you are taking the Xarelto 20 mg daily as prescribed then you should be adequately anticoagulated.
