Upper back pain can be caused by different conditions.
The most common cause is just ,muscular pain or a pain caused by spine problems. These pains are sharp, they can worsen with change of posture, movement and respiration.
Heart attack pain usually is retrosternal, can radiate to back, left arm.
It is a constant pain. It can worsen and can be caused during and after physical activity.
If pain is relieved with aspirin, it is definitely not a cardiac pain.
