Question: I am writing for a girl that lives in the country of Colombia. She is 4. Today a white spot developed on her pupil of her eye, and she cannot see out of that eye as of today. She will be going to the hospital in the morning. What do you think?

There are a number of possibilities to consider

Hi,

The white spot is possibly a corneal opacity. The cornea is the transparent portion in front of the eye, from which you can see the iris behind. If the corneal opacity obstructs light from entering the eye, she would naturally not be able to see. A corneal replacement would be needed if that is the case. But even that cannot ensure her vision since this needed to be done quite early and 4 years is a long time. Lack of vision should have prevented fixation of vision from occurring and this would be a major setback.

The other option to consider is an intraocular mass like retinoblastoma which can be seen through the pupil as a white spot. In that case surgical intervention would be needed. But restoration of vision might be difficult.

The third possibility is congenital cataract. Once again it calls for surgical intervention. Nevertheless, if fixation of vision is lacking, it would still be problematic.
