Is Absence Of Fetal Cardiac Activity In 7th Week Of Pregnancy Normal?

Question: Hello, my LMP is 18/11/2015. I visited for scan on 20/1/2016 for USG. It revealed early intra uterine pregnancy of 7wks 2days with no cardiac activity. It was done as TVS Ultrasound. I have been asked to continue Progesterone and HCG support which I was advised being 32yr old and hypertensive.
I was amlodipine 5mg twice a day dose prior to confirmation of Pregnancy, which was replaced by Labetol 100mg twice/day. As off am on below mentioned medication :
1. Tab. Labetol 100mg twice per day
2. Tab Sustain Once a day
3. Inj. Maintain - Every Wednesday
4. Inj. HCG 5000 units - Every Saturday
5. Tab. Folliden once a day

I have been advised to wait for 10-15 days more and after that they will repeat ultrasound.
Meanwhile am suppose to continue above medicine.

In case, if it is missed abortion, than how will I bleed ? as am hormone support ?
So far I don't have any episode of spotting or bleeding.

Please advice.

Hi Mam,

I can understand your concern.

No fetal cardiac activity in the ultrasound at 7 weeks 2 days needs a repeat scan after 5-7 days to look for cardiac activity if any.

If still there is no fetal cardiac activity, then it indicates Missed abortion or blighted ovum.

Spontaneous bleeding and expulsion of contents will occur in 50 % of cases. If bleeding did not occur, dilatation and curettage will be done.

Even though you are on hormone supplementation, you may get bleeding due to endogenous hormone withdrawal.

Hence you may present with bleeding or even may not bleed at all.

So continue with your present medication.

Repeat scan after one week.
