Brief Answer:
You need to reevaluate the size of enlarged prostate

Detailed Answer:
Hello
Thanks for query ,
Based on the facts that you have posted you seems to have obstructive voiding symptoms due to Enlarged Prostate since 5 years and have been taking Flomax 0.4 mg once daily ,

The maximum recomonded dose of Tomsulosin is 0.4 mg .It will not help to improve urine flow even though you take 0.8 mg of Tomsulosin once daily .

The poor flow in spite of taking Tomsulosin is suggestive of increase in size of prostate over a period of 5 years and needs to reevaluate .

Consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm about the present status of Enlarged Prostate with Post Void Residual Urine (PVR)
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Serum PSA.

If situation demands I would suggest you to undergo resection of Enlarged prostate by an endoscopic procedure called Trans Urethra Resection of Prostate (TURP).This is a gold standard procedure practiced all over the world to resect enlarged prostate It gives permanent relief and helps to improve quality of life at this age .

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.

Brief Answer:
It will not affect erection .

Detailed Answer:
Hello
Thanks for follow up .

TURP is an endoscopic surgery and done under anesthesia .There is no severe pain post imperatively except minor discomfort for a week and do not need to take opiates at all .
However since bladder neck is also resected along with prostate you will have retrograde ejaculation .It will not affect climax of sensation of pleasure of sex (Orgasm ) as usual .

Your Urologist will explain you all the details before surgery .
Thanks and Regards.
