Posted on Fri, 24 Oct 2014
Question: My father has a ruptured ear drum from many years ago. He started having yellow discharge from it a few days ago. I put him on tobradex ear drops twice a day since sunday. He will start ciprodex as of today. Should I also get him on oral antibiotics such as ceftin or just wait and see. He is 89 years old.

Brief Answer:
Needs ent consultation.

Detailed Answer:

Hi,

I had gone through your question and understand your concerns.

I see similar cases amongst old people who visit my clinic, I would like to ask you few more details.

Do you have any other symptoms associated with ear discharge like pain, foul smell in discharge or blood in discharge.

If so, with foul smell discharge, mixed with blood, needs to be evaluated for cholesteatoma.

Cholesteatoma is a bone eroding disease needs good antibiotics and surgery.

In case of simple perforation with ear discharge, you need to protect his ear by not pouring the water. avoid cold or sinus infections.

I suggest you to to consult ent surgeon, get detailed Ent examination. let him differentiate between simple or cholesteatoma ear discharge.

I suggest you to stop tobramycin drops, is a toxic drop for inner ear, he may have giddiness or hearing loss. most of the ear infections are mixed with fungal infections. your treating doctor will decide the type of ear drops and type of antibiotics after the examination.

Hope this answers your question, if you have additional questions or follow up questions then please do not hesitate in writing to us. I will be happy to answer your questions.

Wishing you good health.

Thank you for letting me know about tobramycin. He will be starting on ciprodex as soon as it is delivered. The discharge is clear liquid and yellow once dried up; there is no blood and zero neck pain or actually even ear pain. I assume for now we will start him on ciprodex and oral antibiotics such as Ceftin is not needed?
