Question : my blood sugar is 407. Its about 2 hours after I've eaten. I am stiff and dehydrated. can I go to sleep?

When I see someone like you in my practice, I advise them to immediately go to the ER.
There are certain worrisome complications of diabetes called Diabetic Keto Acidosis (DKA) and Hyperglycemic Hyperosmolar State (HHS).

It is important to rule these out by thorough physical examination and laboratory testing.

After these conditions are ruled out, I would typically order the following blood tests:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
HbA1c (Glycosylated Hemoglobin = your 3 month glucose average)
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH
12 hour Fasting Lipid profile
Urine albumin to Creatinine Ratio
25 hydroxy Vitamin D
