Brief Answer:
Repeat FOBT and Rule out Local causes at first.

Detailed Answer:
Hi Sir,

I understand your query and concern.

Ileocecal TB in my opinion has no bearing on a positive FOBT since more than 10 years have elapsed and complete cure would been possible.

Positive FOBT has to thoroughly investigated only after excluding the more common local causes like hemorrhoids/piles and anal fissures which are more common in your age group.

Since you are currently doing well ,the need for total colonoscopy is very marginal at the moment.

Before thinking of total colonoscopy,i advise you to repeat FOBT after 2 weeks of bed time laxative ispaghula.

A per rectal examination by your surgeon will be helpful to rule out local causes like piles and fissures.

A need for colonoscopy would arise only if FOBT is persistently positive and local causes are ruled out on PR examination.

Thank you.
