It is a good thing as there will be a good HLA (genetic match), so the kidney will have a good life and the requirement of immunosuppression during and post-transplant will be lesser.
Lesser creatinine is seen in individuals with less body muscle. This is not an issue and he can safely give his kidneys to his sister. Increased protein loss in urine and raised blood creatinine is a contraindication to donate kidneys. If all other parameters like blood sugar, HIV,HbsAG, Kidney size, etc are normal then he can safely donate kidneys
