Question : Hi I am on holiday in China. My tour guide has organised a bus, walk and cable car ride to 3100 meters. We are meant to stay overnight there. I previously said not Tibet because of concerns about altitude sickness. I have 2+ heart attacks, blocked right coronary artery and stent to left anterior descending and circumflex. My ejection fraction was 52 %. Since a trip to XXXXXXX Lanka at 2300 meter, I spent a night short of breath until we descended the next day. I was found to have multiple pulmonary embolism on return to Australia and now my ejection fraction is 42 %. Do you think I should not go. My email address is " YYYY@YYYY ". No capital "T" at the beginning. Cheers XXXXXXX No lung disease and nonsmoker

Hello!

Welcome on HCM!

I passed carefully through your medical history and would like to explain that facing your actual left ventricular ejection fraction of around 40%, history of mulriple coronary artery disese treated partially with stents implantation, your multiple pulmonary embolism history and what is most important the experience of shortness of breath when undertaking a similar activity my personal professional opinion would not be in favor of taking such a tour.

I am sorry about that conclusion but it is necessary to consider all the potential complications and the possibilities that are offered to manage them in such an altitude (at 3100m high I don't think there are the necessary facilities for properly addressing such cardiac complications.

Hope to have been helpful to you!

In case of any further uncertainties, feel free to ask me again.

Kind regards,

Dr. Iliri

Thank you Dr Illir Sharka. I was expecting that answer and I am comfortable with it. It was to give some advice to my family and the tour group, who could not understand my reluctance. Thank you for your prompt reply. XXXXXXX
