I am Dr.Muhammad Ahmad. I have read your question completely, i understand your concern and will try to help you in best way possible.

HB of 4 is quite bad if i answer with your used word. It does need two things to be done.

1) Tranfuse blood
2) Find the cause and correct it.

At Hb of 4 body organs start to get low oxygen supply and heart starts to get stressed out due to tachycardia and hypoxia it needs to be fixed on emergency basis. Getting transfusions till HB goes beyond 8-10 is the target then work up is done to find the cause of low Hb and efforts are made to correct it.
