Is High Vitamin B12 Level In A Diabetic A Concern?

Question: My mom has diabetes for the last 7-8 years. Her recent reports says Fasting 194 and PP 364. BP is 140/90. Her blood reports were normal except hemoglobin - it was 11.5. She has started taking Lantas one dose of 16 units everyday after dinner. Doctor also prescribed a tablet to be taken before breakfast Metaforte. SHe is taking Rosuvas as well 10 mg after dinner. Her B12 levels are > 2000 and Vitamin D is 6.5. Serum ceratine was 1.4 and liver tests was normal. Why are the B 12 levels so high? Health check up reports otherwise were normal except sugar and cholesterol and mild UTI . Is the high B 12 level a concern? She also has joints pain currently. Otherwise no other discomfort.

Hi XXXX, I have seen the reports. Your mom is 61 years, and been a Diabetic for about 8 years. Her FBG is 194 and PP is 360; urine shows pus cells (infection), protein 3 plus, calcium above normal 10.5; borderline low potassium; urea raised 41; uric acid 7.9 raised; creatinine-1.14 raised, mild hepatomegaly;B-12-more than 2000 D 6.5;
BP-borderline raised.
In short she has uncontrolled Diabetes with Renal failure.
My advise---Buy a glucometer if you already do not have one and strictly monitor her blood glucose until normal. Under your doctor's guidance, increase Lantus by 2 u till sugar is controlled. Instead of metaforte other tablets that are safe for kidneys should be tried.Diet has to be strictly modified for Diabetes, hypertension kidney problem and raised uric acid ( treat raised uric acid with tablets for present). Liver enlarged slightly but functions normal.Take vitamin D treatment. Yes it is not good to have very high B-12. it gets deposited in liver. Stop calcium and B complex if she is taking. Treat her urinary infection.
I hope her weight is normal, if not reduce. Keep blood pressure below 130/80.
I would not worry so much about haemoglobin or joint pains now. You can revert back after reading my advise. Regards

Hello doctor,

renal failure means what? Is it complicated? will taking medicines and controlling diabetes improve the health of the kidneys?

Gilili Prime MT 40 is what she is taking now. Yes, she is taking calcium tablets , we will stop from today. Increased B 12- anything that we can do to reduce it?


The following result we got today along with the UTI report which I am attaching.

Hi XXXX, yes as expected her albumin in urine is also raised, this is also sign of early kidney involvement. Renal failure means Diabetes has affected her kidneys. Yes it is of concern. Yes to a great extent we can reverse it with proper glucose and Blood pressure control. For B-12 you can stop non vegetarian foods, egg yolk, supplements if you are taking. Please go back to your doctor for a review with new Blood glucose reports after a week.
I will also advise consulting a kidney specialist (Nephrologist) so that although it is early renal failure, it can be handled properly. Regards
