Question : Hi Doctor,

I delivered a baby 5 weeks back and am still having vaginal bleeding. It started decreasing last week but has increased a lot than normal since yesterday. Today I have already gone through 5 pads from morning and it is way above normal. Is it a cause for concern?

Hello,

Thanks for writing to us. Followings are my comments:

1) Light to moderate vaginal bleeding may persist for 2-4 weeks in most cases after delivery. However, heavy or continuous bleeding needs to be concerned.

2) It may be caused by uterus that doesn't contract properly, possibly as a result of fragments of the placenta or the amniotic sac that remain in the uterus after birth, an infection, or both.

3) I suggest you to undergo one pelvic ultrasound scan to check for intrauterine remnants or other pathology. In that case, you may need to do a surgical procedure called a dilation and curettage (D&C) to remove it.

In absence of remnants, Tranexamic acid (500 mg) thrice daily for 3-5 days or cyclic low dose progesterone pill can be given to arrest bleeding. Drink more water and take IRON supplements. Kindly, consult with your treating doctor.
