default
Posted on Mon, 17 Oct 2016
Question: Had a laparoscopy and doctor has started a a luprodex injection 3,75 mg on 3 weeks interval ( 3 injections) . Have taken first shot 2 days ago but don't want to continue because I have read of uncomfortable side effects. What happens if I discontinue. Not comfortable about hormonal therapy. What is this first shot going to do to me? My medical history is detailed below.

Brief Answer:
As below

Detailed Answer:
Hello XXXXXXX
Thank you for trusting me with your health concern.
And profound apologies for a late response, as I was engaged in a very prolonged surgery right uptil now.
See, basically Luprodex contains leuprolide, which is a hormone that suppresses the endometriotic tissue.
Now, I need to know the operative notes of the laparoscopy , to know if your endometriosis wassevere or not.
For severe endometriosis, sometimes, all the disease tissue is not removed properly and hence leuprolide is given to suppress the remaining tissue .
Once you send me the operative details, i can tell you if it is okay to stop the injections.
Yes, there are some uncomfortable side effects, but generally it is advisable to take 3 shots following surgery.
Which city do you reside in ?
The first shot may induce amenorrhoea ( absence of periods ) but no other major side effects.
regarding fertility, if the tubes were found to be open on laparoscopy, you may try for IUI ( intra uterine insemination ) for 3 - 6 more cycles.
Take care.
Please feel free to discuss further.

Operation notes:
Right ovarian chocolate cyst 3*2 CMs drained.
Ovary and tubes adherent.
Endometriosis patches seen on uterosacrals and post uterine wall.
Fulguration of patches done -cautery.

I live in XXXXXXX

Is my endo severe? I have absolutely no symptoms. Regular periods of only 2-3 days with not pain as such.

What are my chances of conceiving naturally after this surgery without luprelide ?

And why is IUI necessary?

Hi
Have attached reports to update reports section. Please confirm if you can see them.
Again I wanted to know my chances of natural conception of i discontinue the lupride treatment . My CA 125 test was in normal range (11) and I have no symptoms of endometriosis: periods regular without pain of 2-3 days.
In short I am living a normal healthy life and I don't want to go though hormonal therapy and side effects it entails.
I am also not interested in any assisted reproduction methods as of now, just want to know my chances of natural conception after this lap I had.

Hello again.
Yes, I can see them.
You have uploaded the CA 125 report 5 times ( 5 attachments ) .
I need to know the discharge card and OT notes of your surgery.
Please attach that as I cannot really answer your questions without knowing the details.
I will do my best to clear your doubts but please understand that I need to know the operative details for that.
Waiting for the report.
Take care.

Detailed Answer:
Thank you for sending the reports.
Actually you can even email the reports at YYYY@YYYY and put in the subject - For Reference by Dr. Aarti.
You can use this option if you face any technical difficulty while uploading the reports.
I have gone through the OT notes.
THe left tube and ovary are adherent ( stuck ) but the right tube is normal.
So yes, you can take a chance for pregnancy naturally since one tube is open.
But your age is 32, with advancing age, the eggs in the female reduce in number and quality.
So you should try for pregnancy very fast.
IUI maximizes chances of pregnancy, and adds more success compared to trying naturally.
Also, is your husband's semen analysis normal.
IF not IVF, you should atleast go for follicular monitoring to check whether egg is forming and developing naturally and then rupturing, so that natural intercourse / IUI can be done on the good favourable days .
That treatment can be done for 3 - 6 months before deciding for the next step.
Take care.
