Is It Necessary To Give DTP And OPV Vaccines At 10 Weeks In An Infant?

Question: Hi I need information on interrupted vaccination

There are a few things to discuss. We changed hospital of our daughter 2 months after her birth and the new hospital gave her wrong vaccination (not as per schedule)

1. At birth she was given BCG + OPV + Hep-B at hospital of birth

2. At 6 weeks she was given DTP / DTaP + Hib-1st dose + OPV/IPV + Hep-B 2nd dose (at the same hospital)

3. At 10 week we change dher hospital. As per her original hospital's schedule she was supposed to be administered DTP/DTap + OPV/IPV + Hib-2nd dose

But instead she was given OPV + Hep-B

What should we be doing now

Brief Answer:
Don't worry, she can receive her further vacvination

Detailed Answer:
Hello

Thanks for the query

I have gone through the details.
There is nothing to worry as she can receive her further immunisation as per schedule.
Consult your pediatrician and get her vaccinated for DTP,IPV and Hib.
I just want to let you know that both Hepatitis B and OPV vaccination were to be given at 10 weeks according to previous guidelines of XXXXXXX academy of pediatrics and there is no harm at all if it is given at this age as in your case.
Recently the guidelines have been changed and because of more use of injectable polio vaccine now a days and no extra advantage of hepatitis B vaccine given at 10 weeks both OPV and Hep B vaccines at 10 weeks of age are not recommended now .
I would suggest you to get her immunised for DTP,IPV and Hib now and her next dose will be after 4 weeks.
The next dose of OPV and Hep B will have to be given at 6 months of age.

I hope this answers your query.

Please do ask if you have any more query.

Dear Doctor

Thank you for explaining so clearly.
Have a further question.

She is 12 weeks old now. This interruption happened at 10weeks. Should we go ahead with her 10th week schedule "now" or wait for another 2 weeks for a 4 week gap.

This question is based on your following comment : "I would suggest you to get her immunised for DTP,IPV and Hib now and her next dose will be after 4 weeks."

Thank You

Brief Answer:
You can go for vaccination right now

Detailed Answer:
Thanks for follow up

I meant to say that there must be a gap of 4 weeks between the second and third dose of DTP,IPV and Hib.
So you can go for vaccination right now but as she is 12 weeks old; the date of her next dose of vaccination (3rd dose of DTP,IPV and Hib)will be at 16 weeks instead of 14 weeks.

I shall be glad to help you if you have any more doubts.

Dear Doctor

This is clear now.

I have a few more questions on this Subject

1. As per her schedule my Daughter was given Pentavac instead of separate DTP+Hep-B+HIB. Is it better

2. What is DTP / Dtap : Does it mean both need to be given?

3. Have info that Hep-B and Hib should not be given together in the third slot (10th week). Hib is given in 10th week and Hep-B IN 14th week. As you are aware now - she was given Hep-B by mistake in 10th week. And as per suggested schedule we will be giving her Hib. So is a 2 week gap between Hep-B & and Hib ok?

4. Just a general info I need - is there a difference in content of vaccine between the 1st / 2nd / 3rd dose. e.g. Hep-B 1st dose, 2nd dose, 3rd dose.. etc..

I really appreciate all your support. It means a lot.
Thank You so much.

Warm Regards

Brief Answer:
'w' stands for whole cell pertussis vaccine

Detailed Answer:
Welcome again

Pentavac is a combination vaccine.Combination vaccines are more preferred vaccine as it has a better compliance and avoids the multiple pricks.

DTwP,here 'w' stands for whole cell pertussis vaccine and in DTaP 'a' stands for acellular pertussis vaccine. There is a debate over the use of acellular vs whole cell vaccine. But XXXXXXX academy of pediatrics has recently recommended the use of whole cell vaccine.Pentavac contains whole cell vaccine so it is as per recommendation.

Now coming to third query. You are right according to the guideline the hepatitis B vaccine should not be given at 10 weeks, even not at 14 weeks (recent recommendation is at birth,6 weeks and then at 6 months). But there is no harm if hepatitis B and Hib given simultaneously.Only two live vaccines can not be given together and both Hepatitis B and Hib are killed vaccine. You can get her vaccinated for hepatitis B at 6 months of age.

Hep B is vaccination for Viral Hepatitis B while Hib is for hemophilus influenzae B.So both are not same.A gap of 4 weeks is needed between two SAME vaccine. So Hepatitis B and Hib vaccine can be used irrespective of gap.

The name 1,2,3 is given according to dosage like at 6 week,10 week and 14 week but there is no difference between the content of vials used for vaccination at 6,10 or 14 weeks.

I hope this answers your query.
If you have any further query I shall be glad to help you out.
Regards
