                                    Brief Answer:
Yes it can happen.
Detailed Answer:
Hello,
Thanks for trusting us with your health concern.
It can happen that there is no bleeding or pain during sex for the first time if the hymen is elastic and she has been using tampons or has been masturbating earlier.
This can be a normal physiological variation.

we had sex second time and there was a pain. she mentioned earlier that she puts half of her finger in her vagina during period just to clean it. Is it normal ? do you think all females cleans vagina during period like that ? do you think her vagina loosen up because of that which caused no pain.

There is no harm in cleaning.
Detailed Answer:
Hello.
Thanks for writing again.
Putting a finger to clean vagina after periods is not harmful but is not necessary either. All females do not clean vagina after periods. This can cause some loosening of vagina. It can reduce pain and bleeding during first intercourse.

Yes.
Detailed Answer:
It is normal not to have pain or bleeding during the first intercourse.
Vagina loosens up and becomes well lubricated if there is proper arousal before the actual penetration.
